Slow Wave Activity Enhancement as a Therapy for Levodopa-Induced Dyskinesia in Parkinson's Disease Patients.

Abstract
Levodopa-induced dyskinesia (LID) is a major motor complication of Parkinson’s disease (PD), associated with maladaptive synaptic plasticity and impaired physiological sleep-dependent downscaling mediated by slow-wave activity (SWA). Phase-targeted auditory stimulation (PTAS) during deep sleep is a promising non-pharmacological strategy to enhance SWA and modulate its homeostatic overnight dynamics. This thesis presents a single-case, randomized, double-blind, crossover study in a 66-year-old female patient with advanced rigid-akinetic PD and LID. The protocol included one month of active stimulation (STIM) and one month of sham stimulation (SHAM). Overnight EEG was acquired using the Tosoo AXO wearable device, which detects deep sleep in real time and delivers auditory stimuli phase-locked to endogenous slow waves.
Motor symptoms, sleep quality, and cognitive status were assessed through standardized clinical scales at the beginning and end of each period, while daily motor fluctuations were monitored using an Apple Watch and PD diaries
